Job Summary
Review referrals and assess eligibility in collaboration with the Medical Director. Assign, support, and mentor interdisciplinary clinical staff. Conduct home visits, record reviews, and case conferences to ensure high-quality care aligned with hospice philosophy and regulatory standards. Guide clinicians in establishing therapeutic goals and individualized plans of care, while leading QAPI and performance improvement initiatives. Support hiring, onboarding, and ongoing staff development in a hands-on leadership role focused on mentorship and clinical partnership.
Required Qualifications
- Licensed RN with current Missouri licensure
- Hospice or home care experience
- Strong in pain and symptom management
- Knowledgeable in grief, anticipatory loss, and end-of-life dynamics
- A confident communicator who leads with clarity and compassion
- Understands that leadership in hospice means supporting both patients and the clinicians who serve them
- Hospice philosophy & family-centered care
- Interdisciplinary team coordination
- Regulatory compliance & standards of care
- Conflict resolution & complaint management
- Staff development, education, and mentorship
- Ethical clinical practice
- Continuous quality improvement (QAPI)
