Clinical Director

Direct and supervise all clinical hospice services and patient care activities, ensuring compliance with Medicare Conditions of Participation, state regulations, and agency policies. Provide leadership, coaching, and performance management for interdisciplinary clinical staff while overseeing patient care planning and team meetings. Monitor quality assurance and performance improvement initiatives, manage clinical staffing and resource utilization, and investigate patient, family, and staff concerns. Collaborate with physicians and community partners to ensure continuity of care and support census growth through clinical excellence and referral relationships. Maintain accurate documentation and adhere to electronic medical record requirements.
