Job Summary
Coordinate medical appointments for developmentally disabled individuals, including scheduling audiology, dentistry, and physical therapy visits. Serve as a liaison between the facility nurse, psychologist, and residence manager to ensure timely follow-ups and accurate medication tracking. Prepare clinic consults and maintain medical documentation using Precision Care software while monitoring bowel movement charts and menstrual cycles. Maintain sufficient medication supplies and equipment, lifting up to 25 pounds and operating Hoyer lifts as needed. Report critical changes in medical conditions or emergency room visits to management immediately. Complete mandatory orientation, First Aid, CPR, AED, and AMAP certification within 60 days.
Required Qualifications
- Valid Driver's License
- HS Diploma/GED
- Proficient reading and writing skills
- Effective communication skills
- Knowledge of basic medical terminology
- Good organizational and time management skills
- Basic computer skills
- Valid AMAP certification
- Good employment standing
- Successful completion of mandatory orientation
- Must successfully pass First Aid, CPR and AED
- Must successfully complete AMAP training along with AMAP Certification as provided by the New York State Office for Persons with Developmental Disabilities (OPWDD) within (60) days
- SCIP training must be successfully completed within (30) days of employment
- Must attend in-house shadowing/orientation
- Must be able to lift and transfer up to 25 pounds
- Must be able to operate Hoyer lifts if needed
- Must wear agency approved face coverings/masks when entering the building and working within the program at all times
- Must participate in daily screenings prior to entering the building
- Must be willing to frequently wash & disinfect hands
- Must be willing to clean AND disinfect frequently touch surfaces
- Must maintain social distance of six feet between themselves and others when appropriate
- Must be able to handle aggressive and/or violent individuals
- Must be willing to assume the risk of working with individuals who may become aggressive and/or violent
