Job Summary
Educate the interdisciplinary team on dietary and nutrition guidelines for older adults with chronic illnesses such as diabetes, CKD, and heart disease. Obtain and evaluate patient nutrition histories, assess knowledge of diet and health maintenance, and provide detailed education regarding nutritional status, fluid status, and lab parameters. Complete written nutritional assessments and comprehensive patient assessments for care plans, then monitor adherence to therapy and adjust treatment as needed to sustain effective nutritional status. Document patient responses in progress notes and participate in care plan meetings. This role supports high-risk, complex care populations virtually, focusing on weight loss, gout, and kidney stones within an integrated specialty care model.

Desired Qualifications
- GA licensure
- Licensed Dietitian Member of the American Dietetic Association
- Experience in adult clinical area
- At least 2 years work experience in adult clinical dietetics
- Experience working with patients with complex, unmanaged Diabetes, Hypertension, and other chronic illness
- Excited and energized by the prospect of working collaboratively as part of an innovative team
- Prior healthcare experience serving chronically ill patients living with limited resources and/or mental illness
- Virtual (phone or video chat) case management or care coordination experience
- Strong customer-service orientation
