Job Summary
Clinical Dietitian in Outpatient and Home Infusion collaborates with referral sources to coordinate care for new patient referrals, conducts nutrition assessments and creates nutrition care plans, provides nutrition education to patients and caregivers, supports discharge planning, and participates in community education and ongoing professional development. Requires registered dietitian status (CDR) and state licensure; bachelor’s degree in Nutrition/Dietetics; at least 1 year of clinical nutrition experience; strong communication and organizational skills; ability to work in a healthcare setting and adapt education for home infusion therapies.
Required Qualifications
- Bachelor of Science degree in Nutrition, Dietetics or related field
- 1 year clinical work experience with knowledge of normal and therapeutic nutrition, nutrition screening and support principles, and modified diets
- Registered Dietitian status with the Commission on Dietetic Registration (CDR) with the Academy of Nutrition and Dietetics
- Current licensed Dietitian/Nutritionist status with LBEDN
