Job Summary
Provide biweekly individual case management and evidence-based group counseling to up to 20 adults with substance use disorders and their families. Coordinate services with medical and social service providers while maintaining clinical charts, conducting pre-audits, and submitting weekly through quarterly reports to HRA. Check the toxicology portal daily, ensure annual medical and psychiatric evaluations are completed, and make collateral contacts for clients. Partner with the multi-disciplinary team to assess diagnoses, plan treatment, and arrange multidisciplinary interventions.
Required Qualifications
- LP-MHC or LMSW
- Minimum of 1 year of experience working with the SUD population
- Ability to multitask and handle challenging situations in a professional and courteous manner
- Maintain confidentiality of records and information
- Willingness to work as part of a team
- Must be organized, self-motivated, a team player, and computer literate
- Ability to work with a diverse set of clients with differing presentations
- Ability to work professionally and empathetically with a diverse group of clients and staff
