Job Summary
Clinical Coordinator/Navigator serves as a key liaison between patients, providers, staffing services, and internal teams to ensure a positive patient experience and seamless coordination of care. This office-based role handles administrative and patient support tasks such as coordinating documentation, communicating with staffing partners, conducting patient satisfaction surveys, tracking service outcomes, and assisting with care navigation activities. Responsibilities include coordinating communication, sending physician orders and documentation to staffing agencies, monitoring documentation requirements, conducting patient satisfaction surveys, tracking feedback, assisting patients and families with questions, maintaining accurate EMR records, ensuring services are initiated and followed up, onboarding and intake coordination, generating reports on patient satisfaction and service outcomes, and ensuring HIPAA and company policy compliance. Qualifications emphasize a high school diploma or equivalent (associate or bachelor’s preferred), at least two years in healthcare coordination or related roles, strong organizational and communication skills, EMR and Microsoft Office proficiency, ability to manage multiple priorities, and knowledge of HIPAA and documentation requirements.
Required Qualifications
- High school diploma or equivalent required; Associate's or Bachelor's degree preferred
- Minimum two years of healthcare coordination, patient navigation, customer service, or medical office experience
- Strong organizational and administrative skills
- Excellent verbal and written communication skills
- Experience with EMR systems and Microsoft Office
- Ability to manage multiple priorities in a fast-paced environment
- Strong customer service and patient-focused approach
- Knowledge of HIPAA and healthcare documentation requirements preferred
