Job Summary
Clinical Coordinator (LTSS) will collaborate with RN case managers to support access to MVP LTSS resources, act as a bridge among customers, health plan, and community organizations, and engage customers through outreach, education, encouragement, social supports, and advocacy. Responsible for assessing customer needs between formal assessments, informing the RN case manager of status changes, developing culturally appropriate care plan interventions, promoting member engagement, and facilitating access to services to enhance health and well-being. The role emphasizes relationship-building with members, families, and caregivers; completion of follow-up calls and ongoing SDOH assessments; clear communication of health status and service delivery; addressing unmet needs with community-based supports; and supporting performance measurements in partnership with the broader team.
Required Qualifications
- Bachelor’s Degree in a Health or Human Service field preferred or combination of an Associate’s Degree in a health or human services field and 2 years of experience in managed care/health services or 5 years as a service coordinator
- Working knowledge of Medicaid programs
- Extensive knowledge of Medicaid LTSS benefits, community organizations, social services, and public resources
- Experience in Managed Care and/or working with Medicaid or Social Service Agencies is a plus
- Ability to incorporate Evidence Based Innovation (EBI) practices into day-to-day customer engagement
- Ability to collaborate with team to assist and resolve member concerns
- Adherence to New York State Medicaid regulatory guidelines
- Strong communication and organizational skills
- Ability to work independently and manage multiple priorities
- Experience with SDOH assessments and care coordination
