Job Summary
Clinical Coordinator I will carry a small caseload and work closely with the Building Manager. Responsibilities include supervising social work and case management staff, providing clinical supervision as needed, reviewing applications for admission as part of the Intake Committee, participating in home visits and admission decisions, supporting psychiatric emergencies, reviewing and signing plans and case notes, ensuring chart audits for regulatory compliance, coordinating staff training with the Building Manager, leading weekly team meetings, facilitating quarterly meetings with Building Manager and staff, providing orientation and training for new social work staff, reviewing incident reports, contributing to site audits and Plans of Correction, and supervising social work interns as needed. Requires collaboration with multidisciplinary teams, strong organizational and problem-solving skills, and a commitment to equity and working with vulnerable populations.
Required Qualifications
- Masters Degree in Social Work (MSW) required
- LCSW preferred
- Minimum of 2 years supervisory experience
- Minimum of 5 years working with older adults or people with a history of homelessness, chronic mental health issues or substance abuse
- Ability to collaborate effectively with multidisciplinary teams and external partners
- Strong organizational, analytical, and problem-solving skills
- Commitment to equity, cultural humility, and working with vulnerable and diverse populations
- Experience with clinical supervision and case management duties
- Experience with home visits and intake/admission processes
