Job Summary
Clinical Coordinator to lead outpatient dialysis care for end-stage renal disease patients. Responsibilities include coordinating patient care plans, monitoring outcomes, supervising clinical staff (including PCTs), ensuring safe, compassionate dialysis delivery, and building long-term relationships with patients and families. Requires current RN license and CPR certification, 18+ months RN experience with dialysis experience preferred (6+ months dialysis), ADN is required with BSN preferred; basic computer skills (MS Word, Outlook) and a flexible schedule including weekends and holidays.
