Job Summary
Coordinate the interdisciplinary Plan of Care for assigned patients, families, and caregivers by identifying physical, psychological, social, economic, and spiritual needs. Perform ongoing physical and emotional evaluations, review responses to interventions, and obtain data on factors influencing health status to incorporate into care plans. Provide initial and subsequent visits to patients in homes, hospitals, or facilities via telehealth, teaching families appropriate care techniques. Serve as a liaison between partner facilities and Arkansas Palliative Care, actively participating in interdisciplinary team meetings and case conferences. Maintain up-to-date patient records, document daily care, and report immediately to providers regarding changes in condition or emergencies.
Required Qualifications
- High school diploma or GED
- satisfactory completion of a nursing assistant (CNA) training, licensed practical nurse (LPN), or medical assistant program (MA)
- Certified Nursing Assistant (CNA)
- Licesenced Practical Nurse (LPN)
- Medical Assistant (MA)
- Must be eligible for a Letter of Determination
- successfully complete a skills evaluation test
- Valid Arkansas driver's license

- CPR certification
Desired Qualifications
- Previous experience in direct patient care in a hospital, long-term care facility, physician office, or hospice
