Job Summary
Clinical Consultant Social Worker conducts clinical assessments (interviews, chart reviews) to inform care plans; develops intervention plans via individual/family/group modalities; coordinates population management and outcomes; ensures thorough documentation and regulatory compliance; facilitates linkages to community resources and handles discharge planning. Requires MSW/LSW with Ohio licensure, 1-2 years’ experience, Day shift, 40 hours weekly in Columbus, OH.
Required Qualifications
- Master's Degree in Social Work (Required)
- LSW license in the State of Ohio
- 1-2 years’ experience as an LSW, preferably in a patient-centric setting
- Licensed Social Worker
- Social Work Certification and Licensure Board
- DL-HC - Driver's License only if in a Home Care department
- Associates hired after 01/10/2019 will need to meet current qualifications
