Job Summary
Review and flag all scheduled patient charts using electronic alerts to identify areas requiring provider attention during upcoming visits. Identify clinical opportunities for follow-up care and order diagnostic testing based on predetermined algorithms for conditions like diabetes and hypertension. Manage dynamic variables including fall risk, smoking cessation, BMI, mammograms, and bone density assessments. Maintain communication with providers and healthcare staff to ensure quality, patient-centered care coordination.
Required Qualifications
- Minimum High School diploma or equivalent
- LPN licensed or MA Certification by the state in which employed
- 3-5 years' experience nursing
- Experience utilizing an EMR system
- Ability to work independently with strict adherence to protocols in an accountable manner
