Clinical Case Manager

Job Summary
Provide case management with crisis intervention, discharge planning, group and individual work, and mental health/substance use disorder counseling while assisting clients in meeting basic needs and symptom management. Initiate referrals to next-level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients regarding medical, educational, social, vocational, and housing resources. Complete accurate documentation to meet billable requirements, facilitate placements including pre-placement visits and transportation, and provide field-based services as needed. Maintain client confidentiality, participate in scheduled program meetings, and support clients in court proceedings as required. Shift coverage includes part of the on-call team.
Required Qualifications
- Master's degree in a behavioral science
- valid/current BBS registration or license (MFT, LCSW, LPCC)
- Valid CA driver's license
- current auto insurance per contract requirements
- Cultural competency understanding
Desired Qualifications
- Experience providing mental health and/or substance use disorder services
- Bilingual
- Knowledge of Microsoft Office
- EHR software
