Job Summary
Clinical Case Manager (RN) in the Tampa region responsible for direct provision of services that link patients and their families to health care, home and community-based services to ensure timely, coordinated access to appropriate levels of care. Requires assessment, planning, coordination, advocacy, education, monitoring, and accurate documentation across program areas, with ability to work with age-specific patient populations. Must hold an active Florida RN license and have 3+ years of clinical experience with 1+ years in case management; strong computer skills (Excel) and EMR familiarity (Athena preferred). Required Qualifications
