Job Summary
Coordinate Individual Support Plans (ISPs) and Individualized Treatment Plans (IDT) meetings, ensuring implementation aligns with written directives. Conduct announced and unannounced visits at residential homes to verify policy adherence and program data accuracy. Analyze incident reports to develop root cause analyses and response plans, while documenting all communications with support coordinators and staff. Provide coverage for direct support personnel shortages and participate in on-call rotations including weekends and holidays. Train direct care staff, conduct internal peer reviews, and maintain the accuracy of personal health records.
Required Qualifications
- Bachelor's degree in Social Work, Social Services, Human Services or Special Education
- Minimum 2 years experience working with persons with intellectual and/or developmental disabilities
- 1-year experience if paired with Master's degree
- Working knowledge of applicable regulations that apply to our service offerings
- Valid driver's license, auto insurance, and reliable transportation
- Must be at least 18 years of age
- Must be able to lift 30 lbs
- Must be able to lift a minimum of 15 lbs
- Must be able to pull minimum of 30 lbs
- Must be able to squat, kneel, crawl, crouch, climb, and stoop
- Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls
- Required to regularly stand and walk
- Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting
- Vision requirements include close vision, distance vision, and peripheral vision
- Must be able to talk and hear

- Drug testing

- Credentialing based on position requirements
