Job Summary
Provide and coordinate ongoing case management for adults with severe and persistent mental illness within a multi-disciplinary team; coordinate housing retention plans with Scattered Sites Supportive Housing; conduct psychosocial assessments, develop consumer-driven goal planners, and facilitate linkages to psychiatric and substance use treatment, 24-hour support services, and access to food, clothing, housing, medical care, financial assistance, and vocational training or employment. Advocate for clients’ access to community resources, maintain professional relationships with landlords and property managers, conduct home visits and housing-related inspections, perform crisis intervention and risk assessment, provide counseling as qualified, and serve as protective payee for some consumers to manage monthly budgets and purchases. Maintain client records, participate in trainings and quality assurance activities, and support clients in achieving and maintaining healthy living conditions through coordination with housing staff and care conferences.
Required Qualifications
- Bachelor’s degree in social work, psychology, or related behavioral science, OR a combination of 1 year of relevant paid work experience and demonstration of the ability to perform required job duties.
