Job Summary
Clinical Case Manager responsible for full-spectrum clinical case management for individuals with chronic behavioral health conditions, including co-occurring disorders, within the CHARTER Program. Responsibilities include therapy case management, resident assessments, individualized service planning, facilitating psychotherapy groups and life-skills classes, crisis intervention, and collaboration with interdisciplinary teams to coordinate housing, healthcare, employment, and education resources. Requires HIPAA-compliant documentation in EHR, trauma-informed care approaches, and active licensure in Utah (CMHC, LCSW, or LMFT) with prior case management experience. Salaried position with benefits and potential merit increases; typical schedules Monday-Thursday or Tuesday-Friday in Salt Lake City, UT.
Required Qualifications
- Master’s Degree Social Work, Mental Health Counseling, Marriage Family Therapy
- Active Utah License (CMHC, LCSW, LMFT)
- Prior Case Management Experience Required
- Experience Working With Chronically Mental Populations Required
- Experience Substance Disorder Treatment Preferred
