Job Summary
Provide field-based services to currently homeless clients, conducting risk assessments and clinical evaluations for those potentially gravely disabled. Engage clients in developing mutually agreed-upon goals and confidential service plans while linking them to medical, mental health, and vocational resources. Assist clients in obtaining public benefits such as SSI, Medi-Cal, and housing applications, and build supportive peer communities. Collaborate with police, hospital staff, and county resources to reduce ED visits and ensure high-level clinical oversight within the Santa Monica Homeless Multidisciplinary Street Team. Complete charting in a timely manner and work effectively on an interdisciplinary team.
Required Qualifications
- Master's degree in a mental health field (MSW, MFT, PhD, PsyD)
- Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record
- Demonstrated knowledge of clinical case management
- Experience in dealing with issues faced by population served (serious mental illness, substance abuse, and physical health problems)
- Excellent writing and charting skills
- Knowledge of community resources
- Able to maintain an effective working relationship with Santa Monica community partners (including police, firefighters, city staff, etc) and maintain and build professional rapport
- Ability to self-motivate, work independently, and communicate effectively
- Able to work collaboratively as part of an interdisciplinary team
- 100% in person by JD
- Master's Degree
- Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs
- On occasion walk or drive to different local sites
- Will necessitate working in busy and loud environments
- Will be exposed to elements like cold, heat, dust, noise and odor
- May need to bend, stoop, twist, and sit throughout the day
