Job Summary
Coordinate care for individuals with chronic or complex conditions by assisting with Transitions of Care post-hospitalization. Work collaboratively with healthcare providers, the interdisciplinary team, patients, and community health partners to manage high levels of care coordination. This role requires an LPN or CMA/RMA with a minimum of two years of clinical practice and current certification from an accredited organization. The position offers daytime shifts from 8:00am to 5:00pm, Monday through Friday, alongside comprehensive benefits including multiple health plans, retirement matching, and 6.5 paid holidays. Required Qualifications
- LPN from accredited College of Nursing
- completion of an accredited Medical Assistant training program
- Minimum of two years of clinical practice
- Currently certified by American Medical Technologists
- Currently certified by American Association of Medical Assistants
- Currently certified by American Registry of Medical Assistants
- Licensed Practical Nurse
- Currently certified by other accredited certifying organization
- Must apply for vaccination exemption prior to employment if not able to be vaccinated due to medical reasons or sincerely held religious beliefs
