Job Summary
SAGE intake specialists, case managers, psychiatrists, and nurses support adults with severe and persistent mental illness in recovery by coordinating housing, medical care, financial assistance, and vocational training. Responsibilities include multi-disciplinary case management, developing client care and housing-retention plans, outreach and engagement, psychosocial assessments and goal-planner development, coordinating psychiatric and substance-use treatments, providing 24-hour support, advocating for access to community resources, making home visits and conducting housing-related inspections, crisis intervention and risk assessment, counseling, and acting as a protective payee when required; travel within the local area to engage clients and maintain housing stability; collaborate with Scattered Sites Supportive Housing, ensure documentation and adherence to deadlines, and participate in trainings and program reviews. Preferred qualifications include eligibility for a Licensed AAC credential, a Master’s degree in social work or psychology, bilingual capabilities in Spanish and English, bicultural experience, and a Mental Health Professional designation. The role operates within DESC’s SAGE program under a Recovery model and includes union representation with SEIU Healthcare 1199NW.
Required Qualifications
- Ability to meet Washington Department of Health requirements for registration as a Registered Agency Affiliated Counselor (AAC) or any superseding credential
- Relevant bachelor’s degree in social work, psychology, or related behavioral science, OR 1 year of relevant paid work experience and ability to perform required job duties
- Ability to drive to conduct agency related business; valid WA driver's license and insurable driving record
- Interest in working with clients who are difficult to engage in mental health/substance use programs
- Knowledge of Recovery-focused, strengths-based work in mental health
- Knowledge of Harm Reduction strategies
- Ability to work effectively with clients displaying a wide range of behaviors
- Ability to communicate and work with staff from diverse backgrounds
- Commitment to cooperation and continuity across programs and respect for consumers
