Job Summary
Assess behavioral health needs of participants and formulate treatment plans with assigned clients, obtaining supervisory approval before implementation. Provide therapeutic interventions including individual, family, and group therapy, while coordinating services with community agencies and scheduling home visits. Manage comprehensive case management by analyzing progress, modifying plans as necessary, and arranging transportation for participants. Respond to crisis calls and participate in an after-hours on-call rotation, reporting suspected abuse or neglect to the Department of Children and Family Services. Maintain complete documentation of services in paper and digital forms and attend regular supervision staffing.
Required Qualifications
- Master's Degree in Social Work or a related field preferred or Bachelor's Degree in Social Work or a related field with at least two years of relevant experience required
- Valid driver's license, insurance, and a reliable vehicle required
- available at least 2 evenings per week for family sessions
Desired Qualifications
- The ability to communicate effectively in both English and Spanish strongly preferred
- Knowledge of resources in the Cook County area strongly preferred
