Job Summary
Conduct patient assessments and gather pre-admission information to ensure viable candidates for alternative site care. Educate patients, families, and facility staff on Option Care services using sales techniques to facilitate timely discharge and therapy independence. Collaborate with internal partners, case managers, and referral sources to coordinate seamless transitions and execute area business growth plans. Review medical records for insurance details, arrange necessary medical supplies, and maintain confidentiality throughout the discharge process. Travel to hospitals and care facilities to provide hands-on training support and promote services to referral sources.
Required Qualifications
- Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice
- At least 2 years of experience in the healthcare industry
- Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors
- Experience applying knowledge of standard practices for all services offered as well as current relevant and applicable standards (i.e. ACHC, URAC standards)
- Experience providing customer service to internal and external customers, including meeting quality standards of services, and evaluation of customer satisfaction
- Basic PC skills: Able to competently use internet, email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint
- Experience in identifying operational issues and recommending and implementing strategies to resolve and improve processes
- Access to a reliable means of transportation which will enable the incumbents to travel to care facilities, home visits and multiple hospitals
- Valid driver's license
- Proof of insurance

- Experience growing service provider partnerships
