Job Summary
Evaluate, educate, and train patients, caregivers, and facility staff regarding Option Care Health services and products to ensure successful transitions to in-home or virtual delivery models. Conduct patient assessments to determine viability for the OCH care delivery model and coordinate with the sales team to develop strategies increasing transition volume. Provide in-person education, gather pre-admission and insurance information, and facilitate service initiation to support safe home care. Collaborate with facility staff, providers, and internal teams to manage patient transitions from hospital to alternate settings, ensuring timely discharge and continuity of care. Maintain confidentiality of patient and proprietary information while documenting education via OCH applications. Requires an active RN license with at least two years of healthcare experience. 100% local travel to facilities and hospitals is required.
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license
- Minimum of 2 years of experience in the healthcare industry
- Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors
- Experience applying knowledge of standard practices for all services offered as well as current relevant and applicable standards (i.e. ACHC, URAC standards)
- Experience providing customer service to internal and external customers, including meeting quality standards of services, and evaluation of customer satisfaction
- Able to competently use internet, email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint
- Experience in identifying operational issues and recommending and implementing strategies to resolve and improve processes
- Access to a reliable means of transportation which will enable the incumbents to travel to care facilities, home visits and multiple hospitals
- Valid driver's license
- Proof of insurance
- Willingness to obtain nursing licensure in additional states if business need supports and geography aligns with market
- Able to plan, organize and make presentations
- 100% local travel to and from partnerships facilities, community hospitals, and medical practice offices to sell Option Care Services, process referrals and provide live education and training support to patient/caregiver and referral sources
Desired Qualifications
- Hands on home or alternate site infusion or discharge planning experience
