Clinical Care Team LPN

Monitor and manage patient communications, guide patients through their care journey, and facilitate appointments and follow-ups. Assist the centralized FMLA team with provider signature forms, process prescription refill requests using the Prescription Monitoring System, and liaise with the revenue cycle team for authorization updates. Coordinate care for patients with acute symptoms, arrange peer-to-peer conversations with providers, and identify qualifying patients for Chronic Care Management to support ER avoidance. Maintain accurate patient records documenting interactions, care time, and outcomes in compliance with health regulations. Work 5/8.5 hour days, Mon-Fri 8:30am - 5pm, in Bohemia, NY.
