Job Summary
Clinical Care Nurse (RN) role in a clinic-based setting focused on improving patient outcomes through transitions of care (TOC) and reducing avoidable ED utilization. Responsible for proactive post-discharge follow-up, education on chronic disease management (DM, HTN, CHF, COPD), and driving Medicare Advantage Stars and quality performance. Works with interdisciplinary teams to identify care opportunities using platforms like Athena EMR and DataHub, coordinates post-hospitalization needs, and supports CMS regulatory standards while delivering culturally appropriate care and health equity. Requires in-center presence at Corpus Christi clinics; typical schedule Mon–Fri, 8:00 AM–5:00 PM with occasional travel within the market. Must reside in designated market area with valid RN license (BSN/ADN), 3+ years of relevant experience, proficiency with EMR/data analytics tools, and Basic Life Support certification. Bilingual English/Spanish strongly preferred.
Required Qualifications
- Associate's degree in nursing (ADN) -OR- Bachelor's degree in nursing (BSN)
- Active, unrestricted RN license (state specific as applicable)
- 3+ years' clinical nursing experience with exposure to transitions of care, quality improvement, managed care, or population health management
- Proficiency with electronic health records (e.g., Athena EMR), data analytics tools (e.g., DataHub, Compass Rose, SalesForce HealthCloud – per your prior employer's population health tools), and Microsoft Office Suite
- Willing and able to complete and maintain Basic Life Support training
