Job Summary
Senior Clinical Care Navigator who independently manages high-complexity, clinically nuanced patient journeys in Substance Use Disorder and Behavioral Health. Coordinates care across clinical experts, operational teams, and external partners; ensures accurate, timely documentation and proactive follow-up; surfaces insights to improve clinical navigation workflows and patient outcomes; mentors CCN I and CCN II teammates; and models Carrum’s patient-first, clinically grounded approach to navigation.
Required Qualifications
- 4+ years of clinical experience in Substance Use Disorder and Behavioral Health patient care with an LCSW, LCPC, LSW, or LPC License
- Substance Use Disorder certification strongly encouraged
- Nice to have: Spanish Speaking preferred
- Nice to have: Direct patient experience in broader behavioral health including eating disorders, other addictions (gambling, sex, digital), and acute mental health needs
