Clinical Care Navigator, Cardiology

Senior Clinical Care Navigator leads complex cardiology patient journeys and multi-system coordination, manages a dedicated caseload of cardiology patients, facilitates referrals for testing and procedures, and participates in the creation and evaluation of educational materials and clinical care paths. Partners with management on daily operations to improve service delivery and patient satisfaction, serves as a subject-matter resource for the care team on cardiac diagnosis management and quality of life, and drives end-to-end navigation to ensure coordinated, timely care. Requires a BSN, RN licensure, and 8+ years of progressive cardiology clinical experience; strong proficiency in evidence-based practice, cross-functional collaboration, HIPAA compliance, and the ability to lead interdisciplinary teams in a remote-first, startup-style environment.
