Clinical Care Manager

Ensure quality of patient care and service by maintaining compliance with State Practice Acts and agency policies. Coordinate personnel assignments, schedule and conduct nursing assessments for new patients, and perform required reassessments. Supervise Home Health Aides through field visits, verify caregiver adherence to care plans, and investigate clinical issues or policy violations. Develop comprehensive patient care plans, submit medical orders, and complete all required nursing documentation. Collaborate with the office team to resolve concerns and promote the agency through community programs. Provide orientation and in-service education for staff. Spend significant time visiting patient homes and medical centers to monitor care delivery in real-time.
