Clinical Care Manager, RN

Job Summary
Coordinate and integrate patient care across clinicians, caregivers, and providers while managing physician orders, documentation, and workflows within the EMR. Conduct ongoing patient assessments, review clinical reports for infection control and on-call actions, and lead case conferences, recertifications, and discharge planning. Serve as a key liaison between physicians, patients, families, and internal teams to ensure compliance with clinical, state, and federal regulations. Partner with Intake to support timely admissions and reduce referral delays. Support care delivery through in-home visits as needed. This full-time salaried role offers flexibility and generous PTO within a home health environment focused on meaningful connections and patient outcomes.
Required Qualifications
- Active RN license in state of practice
- Graduate of an accredited nursing program
Desired Qualifications
- BSN
- Home Health experience
- Experience in care coordination, case management, or clinical leadership
- Strong clinical judgment and ability to manage multiple priorities
- Confident communicator with the ability to collaborate across disciplines
- Comfortable working independently in a fast-paced, field-based environment
- Proven ability to manage complex patient cases and coordinate across teams
- Confidence making clinical decisions and driving outcomes
- Strong organizational skills with attention to detail in documentation and compliance
- Ability to proactively identify issues and take action
- A natural balance of accountability, leadership, and compassion
