Clinical Care Manager RN Hospice

Job Summary
Coordinate and oversee hospice patient care across the interdisciplinary team, including contracted providers; lead the development, implementation, and updates of individualized hospice plans of care; serve as the central point of communication between physicians, caregivers, patients, and internal teams; support patients and families through education, symptom management, and end-of-life care planning; perform in-home visits as needed to support care delivery and team alignment; ensure compliance with hospice regulations, documentation standards, and quality expectations; participate in IDG meetings and help identify trends or gaps in care, escalating to leadership with recommendations.
Required Qualifications
- Active RN license in state of practice
- Graduate of an accredited nursing program (BSN preferred)
- Previous Hospice or Home Health experience required
- 2+ years of leadership, supervisory, or care coordination experience preferred
- Strong communication skills
- Valid driver’s license, reliable transportation, and current CPR certification
