Clinical Care Manager, RN - Home Health

Job Summary
Coordinate and integrate patient care across clinicians, caregivers, and providers while managing physician orders and documentation within the EMR. Conduct ongoing patient assessments, lead case conferences and recertifications, and review clinical reports to ensure compliance with state and federal regulations. Serve as a liaison between physicians, patients, families, and internal teams, supporting care delivery through in-home visits as needed. Partner with Intake to support timely admissions and reduce referral delays. This full-time salaried role offers flexibility and generous PTO for an RN with home health experience to drive meaningful outcomes.
Required Qualifications
- Active RN license in state of practice
- Graduate of an accredited nursing program

- BSN
- Home Health experience
- Experience in care coordination, case management, or clinical leadership
- Strong clinical judgment and ability to manage multiple priorities
- Confident communicator with the ability to collaborate across disciplines
- Comfortable working independently in a fast-paced, field-based environment
- Proven ability to manage complex patient cases and coordinate across teams
- Confidence making clinical decisions and driving outcomes
- Strong organizational skills with attention to detail in documentation and compliance
- Ability to proactively identify issues and take action
- A natural balance of accountability, leadership, and compassion
