Job Summary
Coordinate and integrate patient care across clinicians, caregivers, and providers while managing physician orders, documentation, and workflows within the EMR. Conduct ongoing patient assessments, lead case conferences, recertifications, and discharge planning, and review clinical reports to take appropriate action. Serve as a key liaison between physicians, patients, families, and internal teams, partnering with Intake to support timely admissions and reduce referral delays. Ensure compliance with clinical, state, and federal regulations. This full-time salaried role offers flexibility and generous PTO for an RN with home health experience to drive meaningful connections and patient outcomes.
Required Qualifications
- Active RN license in state of practice
- Graduate of an accredited nursing program

- BSN
- Home Health experience
- Experience in care coordination, case management, or clinical leadership
- Strong clinical judgment and ability to manage multiple priorities
- Confident communicator with the ability to collaborate across disciplines
- Comfortable working independently in a fast-paced, field-based environment
- Proven ability to manage complex patient cases and coordinate across teams
- Confidence making clinical decisions and driving outcomes
- Strong organizational skills with attention to detail in documentation and compliance
- Ability to proactively identify issues and take action
