Clinical Care Manager, RN - Home Health

Coordinate and integrate patient care across clinicians, caregivers, and providers while managing physician orders, documentation, and workflows within the EMR. Conduct ongoing patient assessments to ensure plan-of-care alignment and lead case conferences, recertifications, and discharge planning. Serve as a key liaison between physicians, patients, families, and internal teams to ensure compliance with clinical, state, and federal regulations. Partner with Intake to support timely admissions and reduce referral delays. Review clinical reports, including occurrences and infection control, to take appropriate action. Support care delivery through in-home visits as needed. This full-time salaried role offers flexibility and generous PTO for an RN with strong clinical judgment and experience in care coordination or clinical leadership.
