Lead a team of clinicians as Clinical Care Manager - RN, supervise direct care staff, manage a caseload of clients, develop personalized treatment plans, coordinate care with external providers, conduct comprehensive assessments, lead orientation and training for new hires, act as liaison among staff and healthcare providers, provide in-field supervision and real-time guidance, participate in quality assurance and performance-improvement initiatives, support on-call operations and ensure accurate documentation.
