Job Summary
Clinical Care Manager delivers integrated, whole person care management for individuals with complex behavioral needs. Responsible for utilization management, care coordination across systems, and collaboration with providers, members, and support systems to improve health outcomes. Functions include reviewing service requests for medical necessity, documenting UM activities, discharge/concurrent reviews, facilitating member-centered collaboration, and providing community-based engagement 6–15 days per month as needed. Works with interdisciplinary teams, engages with providers/stakeholders, monitors treatment goals and length of stay, maintains timely documentation, and supports training and compliance. Requires a Master’s degree in Social Work or related field and relevant licensure eligibility, with at least 3 years of direct clinical experience in behavioral health, plus travel to Philadelphia and surrounding counties. Required Qualifications
- Master’s degree in Social Work or related clinical field required
- Licensed/eligible for clinical licensure (LSW, LPC, LMFT, or LCSW preferred; associate license eligible with 90-day deadline to obtain full licensure)
- PA Child Abuse Clearance required
- Minimum of 3 years direct clinical experience providing behavioral health services to children, adolescents, and/or adults (including practicum time)
- Experience in assessment, treatment planning, and interdisciplinary care coordination
- Proficiency with Microsoft Office (especially Excel) and strong clinical writing skills
- Demonstrated knowledge of behavioral health systems, EBPs, and least restrictive recovery-oriented care
- Ability to travel frequently within Philadelphia and surrounding counties; occasional out-of-state travel
- Ability to work independently and collaboratively in fast-paced environments
- Commitment to HIPAA compliance and patient privacy
Additional Requirements
- Philadelphia residency requirement or willingness to become a Philadelphia resident within six months of hire
