Clinical Care Manager

Clinical Care Manager role delivering longitudinal, integrated care management across levels of care for members with behavioral health needs in residential and long-term care settings. Responsibilities include applying behavioral health service continuums and medical necessity criteria, collaborating with members and providers, ensuring evidence-based treatment planning, and supporting discharge planning, transition to lower levels of care, and removal of barriers to care. Team duties include completing 8-15 utilization management activities daily, coordinating with system partners, participating in case reviews, reviewing referrals for residential levels of care, and maintaining collaboration with providers to develop actionable transition plans. Required skills include strong clinical case conceptualization, excellent verbal and written communication, interagency collaboration, familiarity with behavioral health terminology and medications, HIPAA compliance, proficiency with Microsoft Office (especially Excel), and ability to manage a caseload with independence or teamwork. Philadelphia residency is required within six months of hire.
