Clinical Care Manager - Catholic Home Health

Clinical Care Manager oversees all personnel and patient care services for the home health agency, ensuring safe, high-quality delivery of care and coordinating patient-related services. The role requires an active Florida license in one of several clinical professions and CPR certification. Candidates typically have 1–3 years in a health care facility and 3–5 years of supervisory/management experience in home health or a related field, with strong knowledge of patient quality measures, compliance with medical necessity and homebound criteria, and proficiency with clinical software. Key responsibilities include coordinating patient and personnel assignments, reviewing documentation for accuracy and compliance, coordinating referrals, developing personnel qualifications and policies, collaborating with the QAPI team and leadership, developing and updating individualized care plans, ensuring staff competence and education, enforcing policies, and driving quality improvement initiatives. Additional duties include monitoring outcomes data, coordinating with other program areas, and maintaining ongoing communication with physicians, referral sources, caregivers, patients, and management to facilitate coordinated care.
