Conduct telephonic case management and quality process flow initiatives by collaborating with specialists and community physicians to coordinate home health services. Utilize the nursing process to manage nursing case manager caseloads, providing direct patient care, medication administration, and family education while functioning as a patient advocate for crisis intervention. Assist client care coordinators in prioritizing patients through clinical chart review and triage to ensure timely care starts, then document assessments, interventions, and discharge plans within EMR. Run productivity and clinical reports, conduct periodic audits of patient records for QAPI, and evaluate care practice against satisfaction targets. Mentor nursing staff, identify learning needs, and delegate tasks to ancillary personnel according to agency policies.
