Clinical Care Coordinator

Coordinate services for panel patients facing difficulties navigating the healthcare system and manage specialty referrals. Collaborate with outside service providers to ensure seamless transitions of care and provide patient education while assisting with access to applicable community resources. Support patients in self-care and identify relevant resources within an outpatient setting. This role is part of a multidisciplinary care team at Providence Physician Service Organization, focusing on whole-person care for vulnerable community members.
