Job Summary
Clinical Care Coordinator provides ongoing case management and rehabilitation services for adults with mental illness, coordinating care with case managers, networking with community agencies, and delivering trauma-informed care. The role involves skills training, psychosocial rehabilitation, assessments, and development/implementation of recovery plans, with travel to client locations regionally and occasionally out-of-region for trainings. Responsibilities include coordinating services across offices or community settings, utilizing electronic health records, meeting productivity expectations, collaborating with families, and training team members as needed.
Required Qualifications
- Bachelor’s degree in a human services field from an accredited college or university is required
- QMHP CS status is necessary
- Must have a valid Texas driver’s license, a good driving record for the past three years, and personal automobile liability insurance as required by the state of Texas
