Clinical Care Coordinator

Handle all patient and staff calls professionally, relay messages to care providers, and manage fax distribution and paperwork. Process monitor tracking systems to ensure chart documentation, audit prescription reports, and update shot records. Scan all labs into the Provider PAQ, assist with referrals and PAP tracking, and reschedule missed appointments while maintaining electronic health records. Perform scanning duties and other assigned tasks requiring extended sitting and dexterity. This role supports medical care delivery at One Health Ohio locations, serving as a communication hub for patients, nurses, and doctors.
