Clinical Care Coordinator (LVN) Castle Manor

Coordinate skilled services for residents by assessing, documenting, and managing the discharge process in accordance with state and federal regulations and facility policies. Collaborate with the facility IDT to ensure the resident plan of care is maintained, proficiently apply Medicare Rules, Regulations, and RAI assessments, and complete required charting from admission through discharge. Serve as a liaison with Case Managers, create/discharge care plans, participate in resident care rounds with physicians, and attend resident care conferences to support quality nursing care.
