Job Summary

Oversee an assigned caseload of home care clients by conducting Start of Care visits, client assessments, and supervisory visits while developing and updating individualized Plans of Care. Lead caregivers through orientation, training, and competency evaluations to ensure adherence to care plans, and provide direct client support as needed. Manage scheduling collaboration, staffing challenges, and proactive coverage plans while maintaining compliance with Washington State regulations and agency policies. Serve as the primary liaison between clients, families, providers, and leadership to promote continuity of care. This full-time, 40-hour role requires a CNA/HCA with two years of caregiving experience, a valid driver's license, and participation in a caregiver support on-call rotation.

Required Qualifications

  • Current Washington State Certified Nursing Assistant (CNA) or Home Care Aide (HCA) certification
  • Current CPR/BLS certification
  • Minimum two years of caregiving experience
  • Strong leadership, organizational, communication, and problem-solving skills
  • Ability to independently prioritize multiple responsibilities
  • Valid Washington State driver's license
  • Reliable transportation
  • Willingness to travel throughout the assigned service area
  • Must be able to frequently stand, walk, bend, kneel, and reach
  • Must be able to lift up to 50 pounds using proper body mechanics or assistive equipment
  • Must be able to travel throughout the assigned service area
  • Must be able to perform hands-on caregiving duties as operationally needed
  • Must be available for caregiver support on-call rotation
  • Must be available for shift coverage when necessary

Desired Qualifications

  • Home Care leadership or supervisory experience
  • Nurse Delegation certification
  • Experience completing client assessments and supervisory visits
  • Experience training and mentoring caregivers
  • Experience supporting medically complex clients, dementia, Parkinson's disease, hospice, or developmental disabilities
