Job Summary
Coordinate clinical care administration by managing electronic medical records, data entry, and EMR workflow; verify data accuracy with Clinical Manager(s) and/or Clinical Director; manage mail and provide patient and colleague support via phone, email, and fax; alert management to potential compliance issues; maintain PHI confidentiality; order DME and coordinate exchanges; participate in IDG meetings; perform other office procedures and provide training to new Clinical Care Coordinators in the region.
Required Qualifications
- High School Diploma (College Degree Preferred)
- at least two (2) years health care experience, preferably in hospice
- HCHB/Pointcare knowledge is preferred
- Proficient in MS Office and Google Applications
