Clinical Care Advisor - IDD Team Lead

Job Summary

Coordinate operations and workflows for Intellectual, Developmental & Disability case management activities within the CFSP Foster Care program. Perform case management via telephony and home visits to manage complex cases, oversee healthcare costs, and support Special Programs case managers. Conduct assessments, develop comprehensive care plans, and engage with families and service teams to craft individualized treatment plans. Facilitate health rounds, coordinate social determinants of health, and provide patient education, medication reconciliation, and resource identification. Lead specialty projects, assist with process improvements, and train staff while ensuring departmental compliance through audits. Requires an active clinical license and MS/MA degree; travel within the assigned DSS Region is mandatory, with hybrid work options for those within commuting distance.

Required Qualifications

  • North Carolina residency
  • Travel within assigned DSS Region
  • Hybrid work arrangement (field-based with occasional office attendance)
  • MS/MA in social work, counseling, or related behavioral health field OR degree in nursing
  • Minimum of 7 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
  • OR any combination of education and experience providing an equivalent background
  • Active, current, and valid license as an RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States
  • Ability to practice as a health professional within the scope of licensure in applicable states or territory of the United States
  • COVID-19 vaccination
  • Influenza vaccination

Desired Qualifications

  • Experience with IDD population and 1915(i) services
  • Experience working with Children, Youth, and Families served by Local Departments of Social Services through Foster Care and Adoptive Assistance programs
  • Certified Case Manager certification
  • Experience serving children and youth involved in Foster Care and Social Services
  • Prior experience in managing the delivery of services to meet the needs of children and youth with complex medical conditions
  • Knowledge of resources, supports, services and opportunities required for safe community living for populations receiving in-reach and transition services, including Complex Medical, Behavioral Health, and/or IDD
  • Service delivery coordination, discharge planning or behavioral health experience in a managed care setting
