Clinical Associate, RN - Senior Living

Lead and motivate field staff to provide exceptional care while managing client assignments and ensuring cohesive operations with the Rehab Manager and Director. Review clinical documentation for accuracy and coordinate client care, referrals, and individualized plans of care based on continual assessments. Act as the internal case manager and advocate by collaborating on client status, employee assignments, and expected outcomes. Participate in the hiring process and represent the organization in community and marketing activities. This half office, half field role requires 2+ years of RN experience in home health and offers a Monday–Friday schedule with quarterly bonus incentives.
