Job Summary
Clinical Assessment Manager responsible for conducting face-to-face or telehealth UAS-NY assessments per state guidelines to determine long-term community-based needs, documenting comprehensive assessments, and developing projected service plans. Will communicate with members, families, providers, and care teams, perform in-home assessments where needed, ensure compliance with regulatory standards and plan policies, identify alternative care options, and coordinate with multiple departments to support member-centered care. Requires current NY RN license, a bachelor’s degree in nursing (master’s preferred), and at least two years of clinical assessment or homecare experience; role includes leadership responsibilities and collaboration with care management and supervisor for complex cases.
Required Qualifications
- Current license to practice as a Registered Professional Nurse in New York State required
- Bachelor's Degree in nursing or equivalent work experience required
- Master's Degree in nursing or equivalent work experience preferred
- Minimum two years of clinical assessment, homecare or hospital experience required
- Excellent organizational and time management skills, interpersonal skills, verbal and written communication skills required
- Demonstrated strong relationship management skills, including a high degree of psychological sophistication and non-aggressive assertiveness required
- Demonstrated successful conflict management skills and negotiation of “win-win” solutions required
- Working knowledge of Microsoft Excel, Power-Point, and Word required
- Knowledge of Medicaid and/or Medicare regulations required
- Working Knowledge of UAS-NY preferred
