Job Summary
Clinical Assessment Manager leads UAS-NY assessments (face-to-face or telehealth) to determine member care needs across in-home and community settings within VNS Health Plans. Responsible for applying state/federal regulations, documenting comprehensive assessments, coordinating with families, providers, and care teams, and developing individualized projected service plans. Requires strong nursing credentials (RN in NY) with at least two years of clinical assessment experience, plus organizational, communication, and relationship-management skills. Bilingual Chinese-speaking ability is required; role involves travel within service areas including upstate and downstate counties and may include hybrid in-person/telehealth duties in NYC.
Required Qualifications
- Current license to practice as a Registered Professional Nurse in New York State required
- Bachelor's Degree in nursing or equivalent work experience required
- Master's Degree in nursing or equivalent work experience preferred
- Minimum two years of clinical assessment, homecare or hospital experience required
