Job Summary
Coordinate post-discharge follow-up appointments within a 7-day protocol; create and submit prior authorizations; call in-network providers and specialists to schedule patient appointments; call patients to notify them of scheduled appointments; work independently on a computer; maintain communication with a remote team of coordinators and a team lead; adhere to HIPAA and Optum privacy standards; healthcare or care coordination experience is helpful; license/certification not required; remote/telecommute setup in the United States; typical business hours 8:00 AM–5:00 PM PST with a lunch break; interview via video; contractual term slated for 06/22/2026 to 09/18/2026.
Required Qualifications
- Experience in healthcare or care coordination is helpful
- Strong communication skills (phone, email, Teams)
- Ability to manage scheduling and coordination tasks
- Attention to detail and adherence to privacy standards
- Proficiency with technology for independent computer work
